Beneficiary Agreement
As a BHMT beneficiary participating in our education and training programs, you'll receive support from dedicated Client Services Teams made up of career coaches, academic success coaches, outreach specialists, and program specialists. These teams are dedicated to helping you achieve your educational and career goals, and ongoing communication is an important part of that support. Please read and respond to any messages from them promptly so we can effectively assist you throughout your program.
- When participating in the Wage Replacement program, check in with your KP HR consultant before you start the program to understand how participation may affect your KP benefits.
- Make sure your courses support your academic and career goals and can be transferred, if needed.
- Show a commitment to successfully completing your program.
- Attend all training sessions and participate actively. Make-up classes may not be available.
- Follow your instructor's directions. The instructor decides whether you pass or fail.
- Keep all scheduled appointments with your BHMT Client Services team members.
- Notify your Academic Success Coach if your grades fall below the required minimum or if you are placed on academic suspension.
- Submit all required forms, assessments, surveys, documents and mandatory check-ins to stay eligible for BHMT benefits.
- Provide grade or progress reports to BHMT when requested. Failure to do so may result in termination from the education/training program.
You will lose your BHMT benefits effective the date that any of the following changes occur:
- You go on leave
- You are terminated from Kaiser Permanente (KP)
- Your job status changes from benefited to non-benefited
- You move to a KP job that is either non-represented or is represented by a union that is not eligible for BHMT benefits
- For Academic and Certification programs, you agree to allow BHMT to use your KP TR funds to pay for eligible program costs on your behalf.
- It is your responsibility to track your TR fund usage on MyBHMT.org, especially if you also use it for professional continuing education.
- For Academic and Certification Programs If you drop or fail a course after the add/drop deadline, or earn a grade below a C, you must complete an Academic Success Plan before enrolling in more classes.
- If you choose not to complete the plan, you will not be eligible to re-enroll in BHMT-sponsored academic programs for 12 months.
- Not meeting your school's required grades counts as an unsuccessful course completion
- For Regional and Multi-Regional Training Programs If you do not successfully complete your training program, you may lose BHMT eligibility for 12 months, unless an exception is approved by the trustees.
By participating in a BHMT program, you give permission for BHMT staff to access and share certain information while you are enrolled. This includes programs such as career development coaching and academic success coaching. BHMT will not share your grades, scores, or other academic results with your employer unless required to process a tuition payment made on your behalf.
BHMT may access or receive the following information:- Details used to check if you are eligible for the program.
- Job-related information shared between BHMT and Kaiser Permanente, including:
- Date of hire
- Job title
- Hours worked per week
- Starting hourly wage
- Current hourly wage
- Leave of absence status
- Employment status
- Contact information
- State where you live
- Salary increases (amount and percentage)
- Termination date (if applicable)
- Information about your education, training, or job may be shared with your school or training provider to help administer BHMT programs.
- Proof of your attendance and academic standing from your school or training provider.
- Dates (but not the content) of your meetings with a BHMT Career Development Coach or Academic Success Coach.
- After program completion, you may be asked to participate in interviews, focus groups, or surveys with BHMT auditors, your employer or BHMT staff.
If you receive Wage Replacement from BHMT (a payment to help replace your wages from reduced worked hours while you attend school), please review the information below:
- Wage Replacement is considered taxable income by the IRS. BHMT will send the full amount to you by check or direct deposit each payroll period, without taking out taxes or withholdings. BHMT will report the payment to KP payroll, which will then take out taxes (federal, state, Social Security, etc.) and withholdings from your regular KP paycheck the following pay period.
- You will see BHMT payments on your KP pay stub as a separate line called “BHMT Stipends” at the time KP is processing the taxes/withholding for them each pay period.
- At year-end, all BHMT payments will roll up together with your regular KP pay in Box 1 of your KP W-2 form. You will not receive a separate tax form from BHMT.
- Talk to your personal tax advisor if you want to change how much tax is taken out of your paycheck. You may need to submit a new W-4 form or state equivalent to KP payroll.
- Make sure your paycheck is still enough to cover things like childcare, loan payments, retirement savings, union dues or other paycheck deductions. If you get your paycheck through direct deposit, monitor your bank account balances to ensure you have sufficient funds to cover any automatic bill payments.
